    The Impact of Varicose Veins on Health: Insights and Solutions from Singapore’s Specialists

    Varicose veins refer to enlarged, twisted veins usually appearing dark purple or blue on the skin’s surface. A varicose vein often appears as bulging, twisting cords and are commonly seen on the thighs, back of the calves, or the inside of the leg. As people age, the valves in the veins may weaken, leading to blood pooling in a vein. This, in turn, may cause the vein to stretch and twist, thus leading to varicose veins. These can also cause symptoms such as aching, cramping, swelling, and pain in the legs. More severe complications include leg ulcers, changes in skin pigmentation, and blood clots. While anyone can develop varicose veins, they are more common among older adults, women, obese or overweight individuals, and those with a genetic predisposition. Fortunately, treatment of varicose veins is available in Singapore.

    Additionally, symptoms such as pain, aching, swelling, and physical or emotional discomfort due to the appearance of varicose veins are important factors to ascertain. While many people see varicose veins as a cosmetic issue, in some, they may be indicative of vascular disease and are worth looking into.     Prolonged standing, obesity, and hormonal changes that occur in women during puberty, pregnancy, or menopause appear to be the most common causes of varicose veins. These factors tend to place an increased amount of pressure and strain on the veins, which results in the veins becoming varicose. However, contrary to popular assumption, aging may not be a direct cause but rather a contributor to vein-related conditions. Varicose veins develop when they fail to work properly – this is known as venous insufficiency. In these cases, valves to the veins do not close as tightly, leading to the reflux of blood. When blood cannot be effectively sent back to the heart, it starts to pool in the veins, placing extra strain on these already weakened vessels. The added stress, consequently, leads to the veins becoming swollen and varicose. Genetics play a large role in determining the occurrence of varicose veins. The risk of developing this condition is much greater if family members have had them. Other factors, such as leading a sedentary lifestyle and consuming a diet that is low in fiber, can also increase the risk of developing this condition.

    Prevalence and Risk Factors

    Varicose veins are a widespread problem. They are found in 10-12% of men and 20-25% of women. Of the Singaporean adults, 33.3% have varicose veins, but only about 1% of adults with varicose veins have sought medical treatment. The reason many people do not seek medical attention for varicose veins is that they think varicose veins are due to aging and that there is nothing that can be done. This is despite the fact that varicose veins affect younger people and are common in their childbearing years. This review also looks at risk factors for developing and complications from varicose veins. Knowing the risk factors enables people to be more aware of their potential predisposition to develop varicose veins and to seek more appropriate care when required. Varicose veins are more common as people age. Senior citizens are also more likely to have symptoms related to varicose veins. Women are more likely than men to have varicose veins. Lifestyle factors that contribute to varicose veins include being overweight, being pregnant, and standing for long periods. There is a small association with a family history of varicose veins. Because Singapore has an increasing aging population and an increasing rate of obesity, it is likely that the number of people with varicose veins will increase. Knowledge of the prevalence, risk factors, and treatment of varicose veins will help healthcare professionals anticipate and plan for these changes. It will also allow them to advise their patients regarding the risks of developing varicose veins.

    There are different options available to manage varicose veins. These could range from conservative approaches to lifelong medications, minimally invasive procedures, and highly invasive open surgery, depending on the severity and clinical features of the varicose veins and the resulting symptoms. Conservative management may include the adoption of lifestyle modifications (exercises, weight loss, leg elevation at rest), leg compression therapy (wearing compression stockings, wound dressings), and medication (hot fomentation creams, analgesia, anti-inflammatory drugs, over-the-counter micronutrition supplements). The importance of a careful assessment for all patients by a certified specialist cannot be overstated.
